Caregiver Application
Private Nursing Care is recruiting RN’s, LPN’s, CNA’S and HOME HEALTH AIDES who are compassionate,
professional and dependable wishing to work as independent contractors.
Once you met the registration requirements, you will be available to receive referrals to provide private
duty home care services to clients requesting these services in Indian River County, St Lucie County, Martin County and Palm Beach County.
All caregivers registered with Private Nursing Care are independent contractors and are not employees of Private Nursing Care, Inc.
All independent contractor caregivers are required to register in person by appointment at our office
located at 1705 19th Place, Suite I-3 Vero Beach, Fl. 32960. Please email the documents listed below to
PrivateNursingCareInc@gmail.com. You may call the office on 772-696-1536 to make an appointment and
to request guidance on where to obtain the documents needed.
- Must be 18 years of age or older
- Current Florida Department of Health RN, LPN, and CNA license or proof of a minimum of 40 hours of Home Health Aide Training from an FDOE or DOH approved school or program
- Employment Authorization Document(s) (EAD) to work in the U.S.
- Current Driver’s License or Photo ID
- Social Security Card
- Level 2 Criminal Background Screen
- Free of Communicable disease statement (dated within the last 6 months. May not be signed by a Chiropractor.)
- Current CPR from American Heart Association, American Red Cross, or American Safety & Health Institute
- 1 hour of HIV/AIDS Training
- FREE Initial 1-hour DOEA ALZHEIMER’S TRAINING

pop up. Complete form, confirm correct email, then submit. An email confirmation will be provided shortly. Typically, the Certificate of Completion is provided within 24 hours.- 2 hours of Self-Administration of Medication Training
- 2 hours of Alzheimer’s Training
- 5-year employment/ contract history
- Current Auto Insurance – if providing transportation in own vehicle
- Current Vehicle registration – if providing transportation in own vehicle
- Employee Identification Number (EIN) (optional)
- Professional Liability Insurance
- Occupational Accident Insurance
- Voided check for direct deposit
